• We will ask you to complete follow-up questionnaires of similar length in 6 months and then approximately every 2 years. If you become pregnant, we will offer you the option to answer additional questions about your pregnancy. We will hold all personal information you provide in the strictest confidence and use it solely for medical statistical purposes. The initial questionnaire will take around 30 minutes to complete.
